Advanced Care Planning Webinar Series

Join us for a three-part webinar series as we discuss the importance of Advanced Care Planning and best practices for engaging patients and families

The Connected Care of Middle Tennessee ACO Palliative Care Taskforce is hosting a three-part webinar series on Advanced Care Planning, featuring VHAN clinicians from the Connected Care ACO.

Though Advanced Care Planning is an important part of a patient’s care journey, it can be a difficult conversation to initiate. During this interactive webinar series, you’ll learn more about normalizing advanced care planning, engaging patients and families in conversations, assessing patient readiness, and documenting and recording patient care preferences.

Each webinar will be approximately 30 minutes, followed by 15 minutes of live Q&A. We encourage you to register for the entire series as the content will be different each time, or you may select the webinars that work best for your schedule. All presentations will be recorded and made available on the VHAN Hub.


Part 1: “Like a Flu Shot: Normalizing Advanced Care Planning”

This presentation will be hosted by Sam Webb, MD, who serves on the Palliative and Support Care team at Williamson Medical Center. Dr. Webb will share key information about:

  • Why Advanced Care Planning is critical
  • Components of Advanced Care Planning
  • When and where to introduce the topic and how to normalize the conversation
