Your Whole Person Care nurse navigator is your “front door” to all physical, mental, spiritual and emotional care available to you. Your nurse navigator (a live person) can help you find care, answer your questions or guide you to helpful resources.

Whole Person Care Navigation Services

Contact your Nurse Navigator when you:

  • Are not sure where to start or which benefits are right for you
  • Need medical care quickly, but it’s not an emergency
  • Need medical care without leaving your home (telehealth or home visit)
  • Need help finding a Vanderbilt Health provider or specialist
  • Want to book an annual wellness visit or preventive screening
  • Need a primary care provider
  • Need mental health support or resources
  • Need help finding care for a chronic health condition
  • Want to talk about a spiritual or emotional concern

What does Whole Person Care Navigation cost?

There is no cost to speak with your Whole Person Care Navigation team. If you need additional services, those will bill through your standard health insurance.

To get started, book an appointment with your nurse navigator or call (615) 421-1711, 8:30 a.m. – 4:30 p.m., Monday – Friday.

For urgent need or needs outside business hours, call (615) 421-1711. Extended telehealth hours are available 6 a.m. – 9 p.m., Monday – Friday. A copay may apply.

“I’m overwhelmed and need help.”

Brooklyn has had a hard time coping with the loss of her mother. She is overwhelmed and has broken her 16-month sobriety.

A colleague suggests reaching out to the Whole Person Care nurse navigator. Brooklyn does and is connected to a social worker that same day. Her social worker tells her about MNPS benefits and other community resources to get her support.

“I think I have a sinus infection, but it’s after hours.”

MNPS clinics are closed for the day, so Anna calls the Whole Person Care Navigation number. She reaches the extended care team who books a telehealth appointment with a nurse practitioner (NP). After an evaluation, the NP calls an antibiotic prescription into Anna’s local pharmacy. The nurse navigator follows up with Anna the next day to ensure she has her medication and understands the follow-up plan.

“My shortness of breath was scary.”

Joe has been struggling with COPD for years. He had a recent hospitalization that scared him. Everyday tasks have become more difficult. Joe is seeking change.

He reaches out to his primary care physician (PCP) at the MNPS Health Care Centers to share that he wants to prioritize his health. His PCP messages the Whole Person Care nurse navigator and requests a telehealth consult for Joe that is focused on palliative care, which is specialized care for people with a serious illness.
