MNPS has partnered with Vanderbilt Health to offer you two innovative programs:

  • MyHealth Bundles offers world-class care for specific medical conditions using a no-cost-to-you, patient-centered approach.
  • Vanderbilt Health nurse navigation gives you access to a nurse navigator who can guide you to the physical, mental, spiritual and emotional care that is best for your day-to-day needs. Dedicated solely to MNPS employees and their family members, your nurse navigator can help you find care, answer your questions or guide you to helpful resources.



The Vanderbilt Health nurse navigation is available to all MNPS employees, retirees and adult dependents, regardless of healthcare coverage. The MyHealth Bundles program is available to active MNPS employees and their dependents enrolled in the Certificated Employee Health Plan, as well as certificated retirees and their dependents under age 65 who are not yet eligible for Medicare and are enrolled in the Certificated Retiree Health Plan.     Vanderbilt Health Nurse Navigation FAQs

    How can I use Vanderbilt Health nurse navigation for my health care needs?

    Your Vanderbilt Health nurse navigator will guide you to the right level of care for your physical, mental, spiritual and emotional health needs. She can schedule an appointment for you to meet with a social worker or a spiritual health professional. She can also refer you to specialists or help you make an appointment with the MNPS clinics, a provider via telehealth, home care services, Vanderbilt Health specialists and more.

    How can a Vanderbilt Health social worker help me?

    The social worker can help you strengthen coping skills and find effective solutions to life’s challenges. They can also help you find the source of stress or problems in your relationships, work or community life.

    The social worker can provide short-term counseling or connect you to longer-term behavioral health care, connect to community resources to help you and your family meet your basic needs, and provide education on topics such as work/life balance, parenting, anxiety and depression, and substance use disorder.

    Who can use Vanderbilt Health nurse navigation?

    You may take advantage of Vanderbilt Health nurse navigation if you’re an MNPS employee, retiree or adult dependent. Spouses and dependents 18 years of age and up may contact the nurse navigator directly to make appointments. Pediatric patients are currently not covered but plans to include them are being explored.

    For non-urgent needs: Book an appointment

    Urgent needs are those that need to be treated soon to manage potential infection, pain and allergies, but are not life-threatening. Examples include bladder infections, cuts requiring stitches, dehydration, earaches, a rash, sore throat or congestion, or sports injuries.

    Non-urgent needs are less-immediate needs such as check-ups or physicals, vaccinations, health advice, medication refills or changes, referral to a specialist and routine tests. These appointments can be scheduled.

    Emergency care is for life-threatening issues such as chest pain, difficulty breathing or severe injury. For broken bones, head or eye injury, loss of consciousness, poisoning or overdose, severe abdominal pain, severe asthma or allergic reaction, severe burns or laceration, traumatic injury or other similar issues, call 911 or go to your nearest emergency room.


    Vanderbilt MyHealth Bundles FAQs

    What are MyHealth Bundles?

    Vanderbilt Health provides employers, employees and patients with an enhanced care experience through MyHealth Bundles.

    This program “bundles” and coordinates all care—exclusively through Vanderbilt Health providers—needed for specific medical conditions using an innovative, patient-centered approach. MyHealth Bundles combine state-of-the-art medical treatment with concierge patient services to ensure that you and those covered under your health plan receive world-class care quickly and easily with no out-of-pocket costs.

    How could the bundles program benefit me?

    Vanderbilt Health cares about you, your experience with our providers and the quality of care you receive. Patients enrolled in MyHealth Bundles will receive a concierge care experience coordinated through a patient navigator. While each MyHealth Bundle will be different, many will include:
    • Simplified scheduling for appointments and medical procedures
    • Telehealth services to supplement in-person appointments
    • More ways to conveniently connect to Vanderbilt Health’s world-class providers
    • Zero out-of-pocket costs to you

    What type of insurance do I need to have to be eligible?

    MNPS employees and their dependents enrolled in the Certificated Employee Health Plan are eligible to participate in MyHealth Bundles by Vanderbilt Health. Certificated retirees and their dependents under age 65 who are not yet eligible for Medicare and enrolled in the Certificated Retiree Health Plan are also eligible. Participants must also meet certain bundle requirements.

    How do I enroll?

    What criteria do I need to meet to be eligible to enroll?

    MyMaternityHealth:
    • 18 or older
    • Currently pregnant and within the first trimester
    • Enrolled employee or covered spouse
    To maximize your benefits in the bundle, it is best to enroll prior to starting your prenatal care. 

    MyHearingHealth:
    • 18 or older
    • Hearing has been checked by an audiologist in the last year
    • Not related to a mechanical/equipment failure in an existing implant

    MySpineHealth:
    • 18 or older
    • Experiencing pain in neck and/or back
    • Pain is not related to:
      • A work-related injury
      • Tumor of the spine
      • Kyphosis or scoliosis (abnormal spine curvature)
      • A surgery you've had within the last 90 days

    MyOrthoHealth – Hip & Knee Osteoarthritis:
    • 40 or older
    • Experiencing pain, swelling, stiffness and/or heat in hip or knee that needs evaluation
    • Pain unrelated to work injury
    • No history of trauma to joint
    • No previous joint replacement in affected joint
    • Interested in managing knee or hip osteoarthritis without joint replacement surgery in the next year

    MyOrthoHealth – Hip & Knee Surgery:
    • 40 or older
    • Interested in knee or hip joint replacement surgery
    • Diagnosis of osteoarthritis in affected joint, or experiencing pain, swelling, stiffness and/or heat in hip or knee joint that needs evaluation
    • Pain unrelated to work injury
    • No history of trauma to joint
    • No previous joint replacement in affected joint

    MyOrthoHealth – Shoulder Pain:
    • 18 or older
    • Specific shoulder pain diagnoses not related to:
      • A previous injury or fracture/dislocation within the last 12 months
      • A previous surgery
      • Work-related injury or motor vehicle accident

    MyWeightLossHealth – Surgical Weight Loss
    • 18 or older
    • Interested in weight loss surgery
    • Not currently:
      • Pregnant
      • Diagnosed with schizophrenia or bipolar disorder
      • Actively being treated for cancer or receiving any type of radiation therapy
    • No history of organ transplants
    • No weight loss surgery within the last 90 days

    MyUrologyHealth:
    • 18 or older
    • Currently diagnosed with or suspect you have a kidney and/or ureter stone
    • Told by a provider in the last 6 months that you’ll need to have a procedure or surgery to remove the kidney stone and/or ureter stone or wish to discuss treatment options

    MyRecoveryHealth:
    • 18 or older
    • Concerned about your use of mind-altering substances, including alcohol, opioids, and other drugs and substances

    MyHeartHealth
    • 18 or older
    • Have been diagnosed with Afib (atrial fibrillation), aflutter (atrial flutter) or SVT (supraventricular tachycardia) or are concerned about abnormal heart rhythm
    • Not previously enrolled in MyHeartHealth bundle and had procedure done
    • Not had a previous heart ablation at Vanderbilt in the last 5 years
    • If experiencing Afib, have not had it all the time for greater than two years
    • Not on active dialysis
    • Have not had:
      • Stroke or TIA within past 3 months
      • Heart Transplant

    MyOncologyHealth
    • 18 or older
    • Have a confirmed malignancy, a highly suspected malignancy or a history of a known malignancy that has returned or changed
    • Not a diagnosis that:
      • Requires “surveillance-only”
      • Is managed only through a specialty group (non-oncology)
    • Not currently enrolled in another MyHealth Bundle program

      What will it cost to participate in a bundle?

      If you meet the criteria to enroll in a MyHealth bundle, you will have a $0 copay and zero out-of-pocket costs.

      Vanderbilt Health nurse navigation is making it easier to get the care you need—when and where you need it. Nurse navigator, Yolonda Powell, RN, is your “front door” to all the physical, mental, spiritual and emotional care available to you.

      As an extension of the MNPS Health Care Centers, Yolonda can help you find care, answer your questions or guide you to helpful resources. Nurse navigation is available to all MNPS employees and their dependents.

      Five Health Care Centers in the Nashville area offer no-copay visits to employees, retirees and their family members covered by the Certificated Employee Health Plan. Visit us at the Employee Wellness Center at Berry Hill, Two Rivers Middle, Taylor Stratton Elementary, Mt. View Elementary or Bellevue Middle.

      The centers provide care for illnesses/injuries, women’s health, annual and sports physicals, immunizations and chronic condition management, as well as access to health coaches. Additionally, the Employee Wellness Center at Berry Hill offers an onsite pharmacy, café and fitness center as well as onsite physical therapy, behavioral health, acupuncture and chiropractic care.

      MyOrthoHealth: Better Patient Experience With No Cost

      MNPS preschool teacher Natasha Massey will tell you: Not all hip and knee surgery experiences are created equal. When it came time to have her second knee-replacement surgery, she signed up for the MyHealth Bundles program, and she said it made all the difference in her care. “When I had this surgery done on my other knee, I was in another facility for two days, but at Vanderbilt it was outpatient surgery,” she says. “I literally walked the same day.”
