VHAN Care Management a Supportive Solution for Lowering Health Care Costs
As HR leaders know too well, high-cost claimants are the largest driver of health care expenses. Even though less than 2% of all claimants fall into the high-cost category—those with severe illness or chronic diseases that propel claims to $50,000 or more per year—they make up about one-third of employer health care spending.
The next VHAN HR forum—June 16 at 12 p.m.—will discuss a supportive solution to help bring down these high costs: VHAN Care Management. The VHAN Care Management team focuses on patients with complex and costly medical needs to ensure they receive the coordinated care they need in between doctor visits and across care settings.
During the webinar, you will:
- Learn how the personalized care offered by VHAN Care Management helps patients get well faster and better manage their conditions, with cost savings the inevitable result
- Hear from VHAN’s clinical Care Navigators—skilled RNs, pharmacists and social workers—about the ways in which they support employees and their dependents, as well as clinicians employed by the health system
- Discover how to tap into Care Management resources
- Have time to ask questions of the experts and share your own successes and challenges
